- 308 Californians with disabilities
- Collected responses from October 2003 through February 2004
- Administered by mail, in-person, telephone, and Internet
- 66 questions covering priorities

- Surveys completed by mail, telephone and Internet
- Surveys identified by alpha-numeric coding
- Incentives to both consumers and ILCs
